PAIN ASSESSMENT

2 Pain Assessment

The assessment of chronic pain was done on my mother-in-law with using of the COLDERR methods. She told me that the pain started a few months ago after she came to visit my family. The results of the assessment: Character: The pain is mostly a sharp type of pain, and she rate it 8 out of 10. Onset: The pain has been reoccurring three year now but has been more steady and worse since she stays home and not walking around our neighborhood due to the cold weather. Location: mostly in the lower back occasionally, its extents neck and down to her hips. Duration: during the evening time till bedtime. Exacerbation: sitting for an extended period. Relief: she always feels better when she uses ice packs or heating pad. Radiation: Radiates to down to her hips and knee joint occasionally I assessed my mother-in-law that she is 70years old; she has been experiencing some lower back pain for some years. I asked my mother in law if she could describe the pain and tell me how it feels like; she stated that it’s a sharp and rate it 8 out of 10. When I asked about the onset of the pain, she said her lower back and neck would hurt, especially after sitting for a long time. Heating pad and icepacks help relieve her of her lower pain. When asked if this pain ever radiates and to where? she said it would occasionally radiate to her neck and hip. The recommended interventions for her lower back pain should address what exacerbates her pain — sitting down for a long time and non-active. One non-pharmaceutical response will include limiting her in prolonged unsupported sitting during the day, switching sitting position often relaxing when sitting, periodically walk around.

Having some soft support or pillow on the chair in supporting of the back pain and elevating feet while sitting can also help a good position and promote lumber support. Other intervention that I would recommend would be to increase her activity level, not being active gradually can lead to loss of flexibility, and then to have more pain. Furthermore, physical activity and strengthening exercise can help strengthen the core muscle of the lower back pain, improve flexibility. Encourage walking around the house; walking on the stairs can strengthen the muscles that support the back and help reduced pain. Using heat and cold pack to assist in teams of pain....
