Therapeutic class of drug: Antiulcer Agents Pharmacologic class of drug: Proton Pump Inhibitor (PPI) MEDICATION (GENERIC name): Pantoprazole (BRAND name): Protonix IV MECHANISM OF ACTION: Suppresses gastric acid production within 15-30 mins, with half-life of 1hr INDICATIONS: GERD, Erosive esophagitis, Peptic ulcer bleed, Pathological hypersecretory condition CONTRAINDICATIONS: Hypersensitivity, Rilpivirine-containing products, Other PPIs PERTINENT LABS TO MONITOR: AST, ALT, Magnesium, Alkaline phosphatase, Bilirubin ROUTES:PO, IVP

RECOMMENDED DOSAGE RANGE: 40 mg(adults up to 8wk), 20 mg(children) Common side-effects

Life-threatening side effects

Hyperglycemia, Hypomagnesemia, Bone fracture, Abdominal pain, Flatulence, Injection site reactions, Headache, Nausea & Vomiting

Nursing Implications/Patient and family teaching:  Notify PCP immediately if rash, diarrhea, abdominal cramping, fever/bloody stools occur  Pt should avoid alcohol, NSAID products  Notify PCP of all OTC meds, vitamins or herbal products  Inform provider if pregnancy is planned or suspected or if breastfeeding

What focused assessment should be performed prior to administration to ensure the medication is safe to give?  Epigastric/abdominal pain  Occult blood in stool, emesis or gastric aspirate WHY? Risk of GI bleeding

Interactions:  May increase methotrexate levels  Hypomagnesemia increase digoxin toxicity risk  Increase bleeding risk with warfarin  Decrease absorption of drugs requiring acid pH  May cause false-positive urine screening for THC
