11.2.1

URINARY KETONES

Aim To provide education to patients and their carers, regarding how to measure and interpret urinary ketones. Background Information 1. Ketones are acids produced from the breakdown of fat. The presence of ketones is highly significant in a person with Type 1 diabetes and must be treated immediately to avoid progression to diabetic ketoacidosis (DKA). 2. Ketones develop due to lack of insulin, with infection or from starvation. 3. Ketone monitoring for clinical purposes is performed according to PNPM 11.2.2 Blood Ketones. 4. Urine ketone testing is a useful, cost effective tool for outpatients with diabetes, therefore practical experience is required during the inpatient stay. When to Test 1.

Newly diagnosed Type 1 and Type 2 diabetes for educational purposes.

Equipment As for routine urinalysis Urine ketone test strips

PROCEDURE

ADDITIONAL INFORMATION

Collect a specimen of urine in a clean container.

In order to make a valid estimation, it is important that the observations are made on a specimen of freshly voided urine.

Take test strip from bottle and replace the lid immediately.

Exposure to humidity may spoil the test strips.

Dip the test strip in the urine. Drain excess urine.

PROCEDURE Compare tests strip with the bottle legend.

ADDITIONAL INFORMATION Allow correct time before reading the result as per instruction sheet.

Record results on Diabetes Ward Monitoring Chart. A medical officer must be notified if the result reads positive.

Prompt treatment of ketosis prevents diabetic ketoacidosis.
