Resuscitation and Defibrillation:

- Priorities in Cardiac Arrest! - Early recognition! - No signs of life! - Signs of inadequate perfusion! - Better to start CPR than not at all! - Early CPR! - Reasons not to commence;! - Mass casualty situations! - Obvious death (rigour mortis etc)! - Injuries compatible with life ! - Lawful direction! - Early defibrillation ! - Post Resus Care! Withdrawing/Terminating CPR:

- General discontinuation (QAS)! - CPR for no less than 20 continuous minutes ! - And all the following criteria are met! - No ROSC at any stage! - Cardiac arrest not witnessed by QAS personnel! - No shockable rhythm! - Rapid discontinuation (QAS)! - CPR may be withdrawn before 20 continuous minutes if all the following are met! - Patient unresponsive and pulseless for >10 min prior to ambulance arrival ! - No bystander CPR! - Signs of life extinct ! - Asystole or Broad complex PEA 20 seconds ! - Check rhythm between shocks ! - Single shock protocol after initial stacked shocks! Airway management and oxygen therapy:

- 100% oxygen via BVM! - No evidence to support use of any airway! - May use OPA early! - No advanced airway attempted in first 6 minutes ! - For this unit:! - No advanced airway or IV in first 6 minutes ! - Delay to CPR or Defibrillation is a critical error! Waveform Capnography:

- Measure End Tidal Carbon Dioxide ! - Routinely employed in cardiac arrest !

- Early detection of ROSC! - Assess quality of CPR ! - Patient prognosis!...
