Republic of the Philippines) _____________,____________)S.S JOINT AFFIDAVIT OF ARREST We the UNDERSIGNED Police Officers, both members of the Philippine National Police, presently assigned at the Police station of Tampilisan , Poblacion, Tampilisan Zamboanga del Norte, do hereby depose and state THAT: 01. We are among the members of Tampilisan Police Station who conducted the hot-pursuit operation located at Poblacion, Tampilisan Zamboanga del Norte. 02.The said operation is stemmed from a call of Ms. Joecel G. Bayubay for an alleged murder case which is received by our station. 03. On the call dated October 22, 2021,for an alleged murder case. 04. Upon receiving the call, our office send police officer to conduct an investigation and after a successful investigation, the suspect was identified and our office immediately planned a hot-pursuit operation. At about 5:00 of October 22, 2021,the undersigned together with the witness proceed to possible place where the suspect might stayed . 05. Upon arrival thereat, luckily the suspect was present there and has been identified directly by the witness. The undersigned together with the other police officers immediately arrested the suspect. Informing his rights ,while commencing the arrest. 06. Suspect was brought to the Municipal Police Station of Tampilisan, Poblacion, Tampilisan Zamboanga del Norte for booking and proper disposition. Likewise the arrested person was apprised for his rights as provided for under the Constitution of the Philippines. 07. We executed this joint affidavit to attest the truthfulness of the forgoing facts and to support the filling of the Criminal Case against ARVIN C. DIVENAGRACIA for violation of Article 248 of the Revised Penal Code of the Philippines (MURDER). AFFIANTS SAYETH NAUGHT. IN WITNESS WHEREOF, we hereunto affixed our signature this 23rd day of October at Poblacion, Tampilisan, Zamboanga del Norte.
