AFFIDAVIT OF GUARDIANSHIP OF INCOMPETENT I, ELOISA A. VILLAMIL, of legal age, married, Filipino and a resident of 169 5 th Ave., Riverside Village, Ortigas Extension, Brgy. Sta. Lucia, Pasig City, after having been duly sworn under oath, hereby depose and say: 1. That I am the legitimate spouse of the incompetent LINDBERGH S. VILLAMIL, who has an interest in he proceeds of the retirement claim/Provident Benefit claim or refund of the contribution with the Home Development Mutual Fund (PAG-IBIG); 2. That the above-mentioned incompetent is under my care and custody and is presently confined at our residence who is afflicted with Atypical Parkinsonism which affects his motor coordination and balance. 3. That I am competent to receive in behalf of the said incompetent the amount of the refund of Pag-IBIG contribution. 4.
