REPUBLIC OF THE PHILIPPINES) CITY OF CEBU ) S.S. AFFIDAVIT OF LOSS

I, Elon Musk, Chief Executive Officer of Tesla Company, of Legal Age, Filipino, Married, born on the 6th of May, 1999, with residential address at House number 9 Tuscania Gardens, V. Rama, Guadalupe Cebu City, Cebu, subscribing under oath, hereby depose and state THAT: 1. I am a holder of a Driver’s License duly issued by the Land Transportation Office with serial number K00-123-32123 expiring on May 6, 2024; 2. Sometime on March 1, 2021, I went to my friend and upon entry, I surrendered to the security guard the abovementioned driver’s license; 3. That after leaving the ABC Hall, I went to the security to retrieve my driver’s license, however, the security guard who has custody over my license could not be found; 4. Upon returning the day after, I discovered that the security guard has misplaced abovementioned license; 5. Efforts were exerted to locate the driver’s license, but in spite of diligent search, it could not be found and the same is now beyond recovery; 6. Said driver’s license has not been confiscated by the LTO, Police or other Traffic Enforcers for any traffic violation; 7. As such, I am executing this Affidavit of Loss to attest to the truth of the foregoing and to support my application for the issuance of a replacement driver’s license in lieu of the one that was lost; IN WITNESS WHEREOF, I have hereunto set my hand this 2nd day of March, 2021 in Cebu City, Cebu, Philippines.
