AFFIDAVIT OF DAMAGE TO VEHICLE I, SHARON R. ALVARADO, Filipino, of legal age, married, and a resident of Block 1 Lot 10 Angelia Homes Purok Dahlia, Visayan Village, Tagum City, Davao del Norte, Philippines, after having been duly sworn to in accordance with the law, do hereby depose and say THAT: 1. I am the registered owner of a vehicle, more particularly described as follows: Model: Nissan Almera 1.5LE 2018 Color: Alpine White Plate No.: LAC 9450 MV File No.: 1101-00000553892 Registration No.: 320981334 LTO Official Receipt: 1423605223 2. On April 1, 2021, at around 5:00 o’clock in the morning, my husband, Rodrigo Alvarado Jr., parked the aforementioned vehicle at Tagum Public Market and thereafter did some errands; 3. After an hour, upon the return of my husband and upon checking the aforementioned vehicle, he discovered that the same seemed to have been vandalized as it sustained several scratches visible on the left side and rear of the car, as well as several dents and scratches on the right side of the same; 4. There was no such damage at the time he parked and left the vehicle before he did his errands; 5. There were no witnesses of the incident neither was there a CCTV recording in the area which can determine who caused the damage to my vehicle. As such, I do not foreclose the possibility that it was sandwiched by other vehicles causing the scratches on both sides and the rear of the vehicle and that the dents on the right side of the car were hit or made by unscrupulous persons or vehicles; 6. I am executing this Affidavit to attest to the truth of the foregoing, in support of the claim against the insurer for the repair of the damage caused to my vehicle, and for whatever legal purpose/s that this may serve.
