PURPOSE OF MEDICATION Expected Pharmacological Action: Acts on the chemoreceptor trigger zone to inhibit nausea and vomiting; in larger doses, it partially depresses vomiting center.
Therapeutic Use :Antiemetics
Complications: CNS: extrapyramidal reactions, dizziness, EEG changes, pseudoparkinsonism, sedation, drowsiness, motor restlessness, dystonia, tardive dyskinesia.
Medication administration PO IV IM PR
CV: orthostatic hypotension, ECG changes, tachycardia. EENT: blurred vision, ocular changes.
Contraindications/Precautions: Contraindicated in patients hypersensitive to phenothiazines and in patients with CNS depression, including those in a coma.
Contraindicated during pediatric surgery, when using spinal or epidural anesthetic or adrenergic blockers, and in children younger than age 2 or weighing less than 9 kg.
Interactions: Antacids: May inhibit absorption of oral phenothiazines. Separate antacid and phenothiazine doses by at least 2 hours. Anticholinergics, including antidepressants and antiparkinsonians: May increase anticholinergic activity and may aggravate parkinsonian symptoms. Use together cautiously.
Nursing Interventions
To reduce hypotension risk, patients receiving IV drug must remain supine and be observed for at least 30 Client Education: minutes after administration.
Advise patient to report all adverse reactions and to immediately report signs and symptoms of tardive dyskinesia and NMS.
Tell patient to avoid extreme heat because drug may interfere with the body’s thermoregulatory mechanisms.
Evaluation of Medication Effectiveness depresses vomiting center.
