Requirements for Petition for Notarial Commission New Commission I. Submission of the following documents for evaluation purposes 1. Verified Petition, stating among others the following: i. Date of birth ii. Place of residence iii. Contact number iv. Roll of Attorney’s number - 75968 v. P.T.R number – CY 2021 # 4820494 vi. IBP Membership number vii. IBP Chapter – Misamis Oriental viii. Petitioner has not been appointed for the same term as Notary elsewhere by another Executive Judge ix. MCLE Compliance – for renewal 2. Certificate of Admission to the Philippine Bar (same with #6 – ANNEX B) 3. Official Receipt for Payment of Profession Tax – City Hall’s Office (ANNEX C) 4. Official Receipt of Payment of IBP Dues (ANNEX D) 2021 5. Certification of Good Moral Character and a Member in Good Standing to be issued by IBP Executive Officers of the Local IBP Chapter where petitioner is applying for Notarial Commission. (ANNEX E, E-1) 6. Certification from Office of the Bar Confidant (OBC) that Petitioner has no pending case (ANNEX F) and Certificate of Membership in the Philippine Bar (ANNEX B). 7. NBI Clearance 8. Three (3) passport size picture (1.5x1.5cm), colored photographs, with name and signature at the bottom part with light background taken within 30 days before filing of the Petition. (ANNEX A, A-1, A-2) 9. Impression/Marks of Official Seal with Affixed Signatures (ANNEX G) 10. If currently employed in a Government Agency, Letter of Authority to Practice/Notarize documents from his/her Head of Office 11. Official Receipt for Payment of Notarial Petitioner’s fee P2,020.00 (ANNEX H) 12. Publication Fee P3,500 – P5,000??? – after evaluation of petition nah? 13.
