Collaboration and Communication Action Plan Template Based on the scenario you read, create an action plan to assist Mrs. Stone and Marco. Use the template below to define one over-arching measurable long-term goal and four measurable short-term goals as well as steps for achieving those goals and reporting progress. Complete rationales explaining your choices beneath the action plan template. Long-Term Goal: To improve the collaboration and communication efforts between Marco, Mrs. Stone and the special education teacher. Improve training and education to parents, staff and paraeducators on the collaborative supports for IEP and gifted students. Collaborate with administration to create refresher training courses throughout the school year to address collaboration efforts between the special education department and the teachers. Provide long-term goal due date.

Action Plan Short-Term Goals and Steps to Achieve the Long-Term Goal Short-Term Goal 1: Collaborate with Mrs. Stone on strategies to address conflicts or challenges and implementation of positive reinforcements so she can communicate these with Marco.

(This goal should support the long-term goal of, and address communication and collaboration between Marco’s gifted teacher and special education teacher, specifying positives in working with the student in their classrooms, as well as challenges that may arise.

Activities/Strategies Review with Mrs. Stone the special education ethical guidelines and goal setting procedures in working with Marco.

Educating Mrs. Stone on ethical procedures will help remind her of the responsibilities she has to help Marco reach his goals.

What activities/strategies will help implement the goal?

Resources Provide Mrs. Stone with information on emotional/behavior disorder management and strategies for behavior management.

Providing the information will help Mrs. Stone understand that having an emotional/behavior disorder can be effectively managed. Strategies for behavior management will benefit both her and Marco.

What resources are needed to implement the goal?

Timeline The action plan should immediately begin, and measured results should be reached within the first couple of weeks.

What is an appropriate timeline to achieve the goal?

Persons Responsible Weekly collaborations between myself and Mrs. Stone will take place to go over behavior management procedures and brainstorm activities to engage Marco.

Weekly meetings will help support Mrs. Stone in dealing with Marco’s disability as well as give her ideas on activities that will help with inclusion efforts.

What stakeholders will be responsible in helping to achieve the goal and how will they collaborate?

Marco’s progress should reflect the efforts put in place.

Evidence of Success Observation of Marco participating in classroom activities and interactions take place with the teacher for the duration of the course.

How will the goal be measured?

Short-Term Goal 2: Help coach Mrs. Stone in effective instructional strategies and behavior management practices that include inclusion and cultural diversity in the classroom. (The goal should support the long-term goal, and address collaborative coaching for the gifted teacher to include research-based instructional strategies and recommendations that relate to Marcos’ disabilities, inclusive learning practices, and cultural diversity).

Activities/Strategies Encourage group projects and pair Marco with model students. Include a lesson that sparks Marco’s interest and his cultural background. What activities/strategies will help implement the goal?

Because Marco is the only Hispanic student and has a disability, he will need to build his confidence and peer to peer relationships through collaboration. (CITE). Being placed with positive peer models will help him understand social cues and accepted practices. A collaborative project that includes his cultural background topic will help to ensure his participation. Marco will be able to share ideas and increase his participation and creativity.

Resources Writing assignment that includes group collaboration. What resources are needed to implement the goal?

Timeline This goal can be reached within the timeframe of the assignment. What is an appropriate timeline to achieve the goal?

Persons Responsible Mrs. Stone will be responsible for the assignment and monitoring Marco’s progress and I will be responsible for helping Mrs. Stone implement strategies for student collaboration.

Mrs. Stone conscious efforts to choose a collaboration project that helps to benefit Marco demonstrates her willingness to help Marco. My efforts will help Mrs. Stone learn collaboration strategies for students with exceptionalities.

Evidence of Success Careful observation to make sure Marco is collaborating and participating in his group. How will the goal be measured?

Short-Term Goal 3: Include the paraeducator in collaboration efforts such as meetings and brainstorming efforts. Coach them on support techniques and behavior management. (This goal should support the long-term goal, and provide collaborative coaching and guidance for the paraeducator to support the gifted teacher and student.)

Activities/Strategies Training session with the paraeducator and have them shadow a skilled professional to gain skills and techniques.

Professional development and coaching helps the paraeducator get a better understanding to help Marco be successful. The additional techniques learned will aide Mrs. Stone in the classroom.

The objectives are to provide direction and guidance to paraeducators, serve as a collaborative resource to colleagues, and implement research-based strategies for communication and collaboration to diverse students with disabilities.
