Generic name: Terbutaline
Drug class: B2 adrenergic receptor agoinist
Therapeutic action – how does it work? Activates beta 2 adrenergic receptors and this relaxes the uterine smooth muscle
Purposes – why is drug given? Can be used up to 24 hours to delay but not to prevent preterm labor Treats wheezing, SOB, and chest tightness caused by asthma, chronic bronchitis
Contraindications/precautions – when would you NOT use?
Preg risk category C, contraindicated in clients who have tachydysrhythmia. Use cautiously in pts who have diabetes, hyperthyroidism, heart disease, and angina
Administration – how is it given? Route, dosage, special instructions:
Subcutaneously
IV
Dose usually 0.25 mg
If there isn’t a significant decrease in contractions within 15-30 min., a 2nd dose of 0.25 may be given
Side effects – undesired effects? tremor, nervousness dizziness headache drowsiness palpitations rapid heart rate shortness of breath
Complications – dangers of drug? Tachycardia, angina, tremors, MI, skin flushing, restlessness, pulmonary edema for mom Increase in heart rate, increased blood sugar levels for baby
Drug interactions – effects on other meds, other meds effects? Beta adrenergic blockers can negate effects of this med MAOIs and TCAs can increase the risk of tachycardia and angina
Nursing interventions – assessments, other actions? Ensure pt knows dosage schedule Monitor heart Don’t give orally to treat preterm labor
Monitoring effectiveness – is it working?
Patient education – special advice? Advise pt not to exceed prescribed dose Prolonged use of the med increases the risk for life threatening heart problems in the mother
