antiemetic CATEGORY CLASS Antinausea, ______________________________________________________________________ PURPOSE OF MEDICATION

Expected Pharmacological Action Blocks serotonin, both peripgerally on vagal nerve terminals, centrally in chemoceptor trigger zone.

Therapeutic Use Prevention and treatment of nausea/vomiting due to chemotherapy or other causes. Also used in prevention or treatment of nausea postop.

Complications Hypersensitivity, acute renal failure, GI bleeding, respiratory depression, or coma. Side effects include anxiety, dizziness, diarrhea, hypoxia, occasionally abdominal pain.

Medication Administration Baseline Assessment: assess degree of nausea, dehydration. PO: give w/out regard to food. IM: inject undiluted into large muscle mass. IV push: give over 2-5 minutes

Contraindications/Precautions Hypersensitivity to this medication, other HT3 antagonists. Cocomitant use of apomorphine. May cause mild to moderate hepatic impairment.

Interactions Apomorphine: may cause profound hypertension or altered LOC. QT interval- prolonging medications such as amiodarone, azithromycin, ciprofloxacin. Herbal: St. John's Wart. Watch labs for increase in serum bilirubin, ALT, and AST.

Evaluation of Medication Effectiveness Relief from nausea/vomiting generally occurs shortly after administration. Monitor daily pattern of bowel activity.

Nursing Interventions Monitor ECG in patients with electrolyte abnormalities. Assess mental status and assess bowel sounds for peristalisis.

Client Education Avoid alcohol and barbituates. Report persistant vomiting and avoid actitivites such as driving until response to drug is established.
