Greetings Affiliate, Please find below the Ballad Health online student affiliation process. The online database is available at any time, by clicking on the below link: https://students.balladhealth.org/ Clinical and non-clinical students completing a rotation at Ballad Health will be required to complete the online process. Clinical placements are to be secured by the school and/or student. The placement request is to be approved before completing the online process. Please follow the two simple steps below, to successfully complete the online requirements:  

First, the student is to complete the orientation Second, the school is to complete the affiliation checklist

Network access and training sessions will be communicated by email to the school, after the two steps have been successfully completed. ID badges are to be made in an HR Department, outlined in the “How to get Student and Instructor Badges for Ballad Health”, upon completion of each step of the outline process. “How to get Student and Instructor Badges for Ballad Health” Effective January 1, 2019, each student and instructor will be required to secure an ID badge at a Ballad Health Human Resources Department. ID badges can be made at the following locations, Monday – Friday between 9am to 4pm.           

An official Ballad Health identification (ID) badge must be worn at all times while completing the affiliation placement at a Ballad Health site. Initial Ballad Health ID badges will be provided at no cost. A fee will be charged for replacement badges at the cost of $15.00. Anyone requesting an ID badge may be required to present current identification or verification of information before the ID badge will be issued. Lost or stolen badges should be reported to Human Resources immediately. Badges must be worn on the front of the upper body with pictures and name fully visible. ID badges must be collected at the end of each semester and redistributed at the beginning of each semester, by the appropriate school faculty member. ID badges are to be collected and returned to a Ballad Health Human Resources Department upon completion of the student’s final rotation at Ballad Health.
