PURPOSE OF MEDICATION

Expected Pharmacological Action Amphotericin B binds with ergosterol, a component of fungal cell membranes, forming pores that cause rapid leakage of monovalent ions

Therapeutic Use Amphotericin B injection is used to treat serious and potentially life threatening fungal infections. It works by slowing down the growth of fungi that causes the infection.

Complications Impaired renal function is a relatively common complication of amphotericin B, as are other renal manifestations, including urinary potassium wasting and hypokalemia, urinary magnesium wasting and hypomagnesemia, metabolic acidosis due to type 1 (or distal) renal tubular acidosis, and polyuria due to nephrogenic diabetes insipidus.

Medication Administration Amphotericin B injection comes as a solid powder cake to be made into a solution and then injected intravenously, usually over a period of 2 to 6 hours once daily.

Contraindications/Precautions Some products that may interact with this drug include: anti-cancer drugs, azole antifungal cidofovir, digoxin, flucytosine, pentamidine, tacrolimus, aminoglycosides.

Nursing Interventions Monitor BUN, serum creatinine, CBC, electrolytes, and liver function test. Monitor cardiovascular and respiratory status, and observe patient closely for adverse effects during initial IV therapy. Monitor I&Os and weight.

Interactions Capreomycin, cisplatin, carboplatin, colistin, cyclosporine, mechlorethamine, furosemide, vancomycin increase the possibility of nephrotoxicity.

Client Education Evaluation of Medication Effectiveness Patient will notice improvements within one to two weeks of taking medication

Notify physician if no improvements within 2 weeks or if lesion appears to worsen. Wash towels and clothing that were in contact with affected areas after each treatment. Topical cream slightly discolors the skin. Do Not breastfeed while on this medication without consulting your physician first.
