Case: Sully
Two Major Entities:

Captain Sullenberger Root Definition Sullenberger ditched the plane on Hudson stream with the assistance of his co-pilot and group members after the both engine failure of plane because of feathered creature strike which spares the lives of 155 passengers, who were supposing they are going to die in light of the fact that it was unrealistic to arrive in closest airplane terminal in brief timeframe just as in less height and risky to arrive in Hudson waterway because of cold weather. The Key verbs that I have identified in this root definition are: Ditch, Fail, Feather, Fear, Reach, Near, save, Strike. The purposeful activity model is shown in the picture below where the three Es are described as follows: E1: Are all passengers safe? As our first E(Efficacy) belongs to the process that weather transformation is working to produce its intended outcome so in this entity the owner sully is checking for the safety of passengers. E2: How quickly sully landed the plane? Our second E which means efficiency judge the ability of sully that weather he has landed on water with the use of minimum resources that he has in successful way or not? E3: Is the water landing best method?
Third E (Effectiveness) that relates with longer term goals. In this the transformation has received some longer-term goals such as they come to know about many facts about the water landing of aircraft.

Root Definition NTSB made a committee of members from FAA, EASA and some other aviation association to analyze the decision taken by captain sully that whether it was write or wrong by checking the facts, following guidelines and running dozen of simulations with the help of simulation staff who run the simulation in the same environment as sully and his team did in reality. If they find that the decision was wrong then it could have the bad impact on airline company, insurance company as well as it could also ruin the career of captain sully and his co-pilot. The Key verbs that I have identified in this root definition are: Made, Analyze, Taken, Run, Bad Impact, Ruin.

E1: does the decision taken by captain is right? In this case our entity is NTSB so the first E is doing the decision taken by captain is right or wrong. This E help us to identity Whether this process gives us desired outcomes. E2: Does simulation results are achieved within minimum use of resources? Run the simulations within desired amount of time and by using minimum resources or setting the same environment as it was in reality. E3: Is the NTSB satisfied? This E is used as a effectiveness measure so is this whole process makes the NTSB satisfied then it can help in achieving long term goals means they can make some changes in new aircrafts to prevent these kind of accidents.
