Expected Pharmacological Action -Cholecalciferol requires activation in the liver and kidneys to create the active form of Vitamin D
Therapeutic Use -Treatment and Prevention of deficiency states, particularly bone manifestations -Improve calcium and phosphorous homeostatis in pts. with kidney disease
Complications -Dizzziness -Malaise -Dyspnea -Constipation -Dry Mouth -Allergic Reaction -Chills -Fever -Angioedema -Arhythmias -Palpitations -Headache
Medication Administration -PO and IV -Daily
Contraindications/Precautions -Contraindicated in pts with Hypersensitivity, Hypercalcemia, Vitamin D Toxicity, Lacting Patients -Use cautiously in pts rcieving digoxin, safety not establshed in OB patients
Interactions -Cholestyramine, Colestipol, or Mineral Oil decreases absoorption of Vitamin D Analogues, use with thiazide diuretics may result in Hypercalcemia, Corticosteroids decrease effectiveness of Vitamin D Analougs, use with Digoxin may increase Arrhythmias, Other Vitamin D Aids
Evaluation of Medication Effectiveness -Prevention of Hypocalcemia -Adequate Calcium and Vitamin D levels -Prevention of deficiency states, and bone manifestations

Nursing Interventions -Check for Vit D defiviency prior & periodically during therapy -Assess for bone pain and weakness prior & priodically -Observe pt carefully for signs of hypercalcemia protecting symptomatic pts by padding bed rails and keeping bed in lowest position -Monitor lab results for Calcium and vitamin D levels
Client Education -Take medication as directed -Do not double up on missed doses -Encourage pt to comply with dietary recommendations -Avoid use of antacids containing magnesium...
