Channel Blockers CATEGORY CLASS Calcium ______________________________________________________________________ PURPOSE OF MEDICATION

Expected Pharmacological Action It's a calcium ion influx inhibitor (slow-channel blocker or calcium ion antagonist) and inhibits the transmembrane influx of calcium ions into cardiac muscle and smooth muscle.

Therapeutic Use Treats angina, hypertension, and dysrhthmias.

Complications Headache, dizziness, fatigue,asthenia, paresthesia, vertigo, peripheral edema, chest pain, hypotension, epistaxis, rhinitis, nausea, constipation, urinary frequency, erectile dysfunction, leg cramps, flushing, rash

Medication Administration P.O.

Contraindications/Precautions -Hypersensitivity to drug -chronic renal insufficiency, hypotension, aortic stenosis, heart failure, significant left ventricular dysfunction (especially with beta-adrenergic blockers), peripheral edema, elderly patients, pregnant or breastfeeding patients, and children

Nursing Interventions -Monitor VS, cardiovascular status, chest, edema, and rash.

Interactions Beta-adrenergic blockers: increased risk of heart failure, severe hypotension, or angina exacerbation Cimetidine: increased nifedipine blood level Coumarin anticoagulants: increased prothrombin time Digoxin: increased risk of digoxin toxicity Quinidine: decreased quinidine blood level

Evaluation of Medication Effectiveness Reduced angina attacks, blood pressure, and amount of dysrhthmias.

Client Education -Can take with or w/o mealscan take with grapefruit juice if GI upset occurs, do not crush/break extended release tablets, angina attacks can occur w/in 30 minutes after dose (this is temporary and does not mean drug should be stopped), avoid driving until pt knows effectiveness of drug, must tell PCP of any supplements or OTC drugs pt is taking, reduce dose in hepatic impaired pt's, can lead to sudden death, hypokalemia or hypomagnesmia (labs should be checked regularly), and be aware if any other signs or symptoms occur/...
