Description of Skill Many medications prescribed for respiratory problems may be delivered via the respiratory system using a small-volume nebulizer. A nebulizer uses the force of an oxygen stream or air compressed through a fluid medication to disperse fine particles of liquid medication into the deeper passages of the respiratory tract, where the absorption will occur
Indications This therapy may be evaluated for patient effectiveness and appropriateness by assessing the patients airways clearance, respiratory effectiveness, and breathing patterns Patients may be contraindicated for various reasons including by mouth restrictions and respiratory inabilities This therapy may be used to provide more thorough and clearing relief to patients who experience oxygen insufficiency and shortness of breath by expanding airways to promote comfort
Outcomes/Evaluation The successful outcome will include that the medication is fully and correctly administered and that the patient receives the desired and expected responses to the therapy while denying any adverse effects The patient will demonstrate correct usage and dosage frequency of the nebulized medication
Potential Complications Adverse reaction to drug Patient may be unable to hold the nebulizer properly or may be unable to keep lips/mouth in the proper position to ensure medication is properly administered
CONSIDERATIONS
Nursing Interventions (pre, intra, post) Educate the patient of proper respirations to facilitate most efficient inhalation of medication and inform them of the proper position to be in to receive maximum effects of medications. The nebulized administration takes as long as there is fluid in the nebulizer cup, observe the patient periodically throughout administration to ensure proper inhalation and to discontinue the therapy as soon as the medication is all absorbed
Client Education Advise patients to take as directed, some vasodilating medications that can be nebulized may cause paradoxical spasm or rebound respiratory congestion when used incorrectly or too frequently Ensure patient understands and demonstrates the correct procedure and body mechanics during reception of this therapy, speaking or eating during administration or laying down during administration is not advised
Nursing Interventions Monitor patient closely for adverse effects throughout and following treatment Ensure a thorough respiratory assessment takes place before and following medication administration and complete all required documentation An oxygen face mask may be used in place of the nebulizer mouth piece as needed eliminating the need for the patient to hold the mouthpiece, monitor for correct mouth placement and that medication is being properly inhaled
