Medication Administration Safety: Scenario: The nurse is caring for a client, Mrs. Goodman, who has an elevated temperature. The nurse administered acetaminophen to the client for an elevated temperature at 38.3º C (101º F).

1. After reviewing the electronic medical record, what potential safety concerns should the nurse be aware of prior to administering other medication(s) to Ms. Goodman? First of all, it is important that the nurse should make sure that other medications that are ordered for Ms. Goodman are the correct dosage, given to the right patient, and administered via the right route. According to the ATI video case study on medication administration safety, the nurse must make sure that medication is administered for the right drug, patient, route, and time. Also, this video case study mentions that it is important to ensure that the patient is not allergic to these other medications. (Assessment Technologies Institute (ATI) (2021)). It is also crucial that the other medications being administered does not interfere with acetaminophen. According to Davis’s Drug Guide for Nurses, there is an increased risk for bleeding when acetaminophen is taken with warfarin. The use of isoniazid, rifampin, rifabutin, phenytoin, barbiturates, and carbamazepine may increase the risk of acetaminophen-induced liver damage and decrease the therapeutic effects of acetaminophen. The use of NSAIDs with acetaminophen may increase the risk of adverse renal effects (Sanoski (2021a)). Since there are many medications that can create harmful effects on a patient or decrease the effectiveness of acetaminophen, the nurse must be knowledgeable of what medications may not be taken simultaneously with acetaminophen.

2. What are several effects of an acetaminophen overdose on the body? There are varying levels of severity when looking at the effects of acetaminophen overdose on the body. According to Abram’s Clinical Drug Therapy, the main concern for acetaminophen overdose is fatal hepatotoxicity. It is stated that “metabolism of acetaminophen produces a toxic metabolite that is normally inactivated by combining with glutathione. In overdose situations, the supply of glutathione is depleted, and the toxic metabolite accumulates and directly damages liver cells,” (Frandsen & Pennington, 314 (2021a)). Considering the severity of this situation, this would be the main concern for an acetaminophen overdose.

Earlier symptoms of overdose are less severe, and these include nausea, vomiting, anorexia, and diaphoresis. Although these symptoms may be mild and vanish over time, it is important to check liver function, as hepatotoxicity can potentially occur. Later symptoms of overdose may include jaundice, vomiting, or CNS stimulation with excitement and delirium. These can be followed by severe consequences, such and vascular collapse, coma, and death. (Frandsen & Pennington (2021b)).

3. Describe several actions the nurse should take if the client displays manifestations of an acetaminophen overdose? According to Davis’s Drug Guide for Nurses, acetylcysteine (Acetadote) is the antidote for acetaminophen (Sanoski (2021b)). This drug is crucial when a patient is displaying manifestations of an acetaminophen overdose. Abram’s Clinical Drug Therapy mentions that acetylcysteine may be taken orally or intravenously, and its function is to supply cysteine, a substance required for the synthesis of glutathione (Frandsen & Pennington (2021c)). As stated above, the depleted levels of glutathione in a patient with an acetaminophen overdose causes toxicity because there is not enough glutathione to bind with the toxic metabolite produced by acetaminophen. Therefore, the toxic metabolites are not inactivated due to the lack of glutathione. However, the cysteine that is present in the acetylcysteine helps to synthesize more glutathione, which would decrease the levels of toxic metabolites produced by acetaminophen (Frandsen & Pennington (2021d)). If Mrs. Goodman were to overdose on acetaminophen, the nurse should administer acetylcysteine. Even in the case of mild symptoms, the liver function must be monitored by the nurse. Abram’s Clinical Drug Therapy states “with blood levels greater than 300 mcg/mL at 4 hours after ingestion, about 90% of patients develop liver damage,” (Frandsen & Pennington, 314 (2021e)). It is crucial that the patient is frequently being monitored so the nurse can ensure that the liver damage is minimal, and admiration of acetylcysteine is given prior to peak liver damage, so some manifestations of acetaminophen overdose is minimal.
