Expected Pharmacological Action It inhibits reflexes at the spinal level.

Therapeutic Use Decreases muscle spasticity, bowel and bladder function can be improved.

Complications Seizures, dizziness, drowsiness, fatigue, weakness, edema, nausea, pruritis, rash, hypersensitivity, sweating.

Medication Administration PO (adults)5 mg x 3 times a day. (children: 8 +) 30-40 mg, children (2-7) 20-30 mg, children (/ 25-1200 mcg/day infusion.

Contraindications/Precautions Patients in whom spasticity maintains posture and balance; Patients with epilepsy; Renal impairment; Geriatric patients are at increased risk of CNS side effects.

Interactions Increased CNS depression with other CNS depressants including alcohol, antihistamines, opioid analgesics, and sedative/hypnotics.

Nursing Interventions Assess muscle spasticity before and periodically during therapy. Observe patient for drowsiness, dizziness, or ataxia. IT: Monitor patient closely during test dose and titration. Keep resuscitative equip handy.

Client Education Evaluation of Medication Effectiveness Decrease in muscle spasticity and associated MS pain with an increased ability to perform activities of daily living. Decreased pain in patients with trigeminal neuralgia. May take weeks to obtain optimal effect.

Instruct patient to take baclofen as directed. Take a missed dose within 1 hr. Discontinue baclofen gradually over 2 wk or more. May cause dizziness and drowsiness. Advise patient to avoid driving.
