Examine risk factors: to identify modifications, adopt mutually agreeable goals and identify support systems - Refer clients to educational support resources Use behavior changes: Identify clients readiness to recv and apply health info, acceptable interventions, help motivate change by setting realistic goals. Reinforce steps to change, Assist the client to recognize their personal barriers. Promote healthy living: Use stress management, get good amount of sleep and rest, eat healthy, avoid saturated fats, physical activity
Tertiary prevention: aims to prevent the long term consequences of chronic illness or disability and to support optimal functioning; Begins after an injury or illness, preventions of pressure ulcers after spinal cord injury, promoting independence
