ANTIHYPERTENSIVE CATEGORY CLASS DIURETIC; ______________________________________________________________________ PURPOSE OF MEDICATION

Expected Pharmacological Action Sulfonamide derivative that causes both potassium and magnesium wastage. Inhibits sodium and chloride reabsorption by direct action on proximal ascending limb of the loop

Therapeutic Use Produces only mild hypotensive effects at usual diuretic doses. Controls formation of edema.

Complications Sweating, hyperventilation, glycosuria, dizziness, headache, weakness, fatigue. Hypotension, ECG changes, chest pain, hypovolemia, Ear discomfort, ringing or buzzing in ears, impaired hearing.

Medication Administration Tablet; injection

Contraindications/Precautions Hypersensitivity to bumetanide or to other sulfonamides; anuria, markedly elevated BUN; hepatic coma; ventricular arrhythmias; severe electrolyte deficiency; lactation.

Interactions AMINOGLYCOSIDES, cisplatin increase risk of ototoxicity; bumetanide increases risk of hypokalemia-induced digoxin toxicity; NONSTEROIDAL ANTIINFLAMMATORY DRUGS (NSAIDS)

Nursing Interventions Monitor for hearing difficulty or ear discomfort. Monitor I&O and report onset of oliguria or other changes in I&O ratio and pattern promptly. Monitor diabetics for loss of glycemic control.

Client Education Evaluation of Medication Effectiveness Less edema and fluid retention.
