Expected Pharmacological Action Decreases neuronal reuptake of dopamine in the CNS. Diminished neuronal uptake of serotonin and norepinephrine (less than tricyclic antidepressants).

Therapeutic Use Diminished depression. Decreased craving for cigarette

Complications Drug-Drug: Concurrent use with MAO-inhibitor like drugs, such as linezolid or methylene blue mayqrisk of hypertensive reactions; concurrent use contra- indicated; do not start therapy in patients receiving linezolid or methylene blue

Medication Administration PO (Adults): Zyban — 150 mg once daily for 3 days, then 150 mg twice daily for 7 – 12 wk (doses should be at least 8 hr apart)

Contraindications/Precautions Hypersensitivity. Concurrent use of MAO inhibitors or MAO like drugs. Interactions Concurrent use with MAO-inhibitor like drugs, such aslinezolid or methylene blue may increase risk of hypertensive reactions

Evaluation of Medication Effectiveness Increased sense of well-being. Renewed interest in surroundings. Acute episodes of depression may require several months of treatment. Cessation of smoking.

Nursing Interventions Monitor for therapeutic effectiveness. The full antidepressant effect of drug may not be realized for 4 or more weeks. Close observation for worsening of depression or suicidal tendencies. Monitor for and report delusions, hallucinations, psychotic episodes, confusion, and paranoia.

Client Education -Advise family and caregivers to observe for increased suicidal thinking and other side effects -teach the patient to report symptoms of serotonin toxicity such as fever, excessive sweating, shaking, diarrhea, loss of coordination-Tell the patient it may take 4 weeks to notice any results and not to chew crush or divide tablet...
