PURPOSE OF MEDICATION
Expected Pharmacological Action helps your body produce a substance that dissolves unwanted blood clots
Therapeutic Use used for acute ischemic stroke, acute myocardial infarction, pulmonary embolism "clot buster"
Complications nausea/vomiting, hypotension and dizziness mild fever allergic reactions
Medication Administration 10% of the total treatment dose should be administered as an initial bolus over 1 minute. The remaining treatment dose should be infused intravenously over 60 minutes.
Contraindications/Precautions History or evidence of ICH. Known arteriovenous malformation. Systolic BP exceeding 185 mm Hg or diastolic BP exceeding 110 mm
Interactions warfarin aspirin NSAIDs
Nursing Interventions monitor closely for signs of bleeding (petechiae) use caution with patient who have had recent surgery may cause intracranial hemorrhage monitor for anaphylaxis or allergic rxn
Client Education Evaluation of Medication Effectiveness no major blood clots
tell patient to contact healthcare for unusual bruising, pink or brown urine, red or black or tarry stools, coughing up blood, vomiting blood or headache
"remains safe and effective when given 3-4.5 hours after AIS onset"
