Expected Pharmacological Action Partial agonist at serotonin 5-hydroxytryptamine1A receptors by binding to serotonin and dopamine receptors in the brain. Increases norepinephrine metabolism in the brain
Therapeutic Use Manage/relief of anxiety. Treatment for hypoactive sexual desire disorder in women.
Complications CNS: serotonin syndrome, dizziness, drowsiness, HA, fatigue, insomnia, excitement, personality changes, weakness, nervousness, lack of coordination, Parkinsonism, numbness, paresthesia, tremor CV: chest pain, tachycardia, palpitations EENT: blurred vision, nasal congestion, tinnitus, altered taste, sore throat, vision changes ENDO: irregular menses GI: n/v, constipation, diarrhea, other abd or gastric distess GU: changes in libido, urine retention, urinary frequency/hesitancy MS: myalgia RESP: SOB, chest congestion, hyperventilation SKIN: rashes, easy burising, ecchymosis, diaphoresis, urticaria, flushing, pruritus, dry skin, blisters other: clamminess, sweating fevere, angioedema
Medication Administration PO: onset- 1-4 weeks, peak3-5 weeks, and durationunknown
Contraindications/Precautions CONTRAINDICATIONS: hypersensitivity, severe hepatic/renal impairment, concurrent use of MAOIs, ingesting large amounts of grapefruit juice CAUTIONS: pts taking other antianxiety agents (taper to prevent withdrawal or rebound) and pts on other psychotropic meds
Interactions DRUGS: MAOIs, erythromycin, nefazodone, ketoconazole, irtraconazole, ritonavir, inhibitors of CYP3A4, rifampin, dexamethasone, phenytoin, phenobarbital, barbamazepine, haloperidol, CYP3A4 inducers FOOD/HERBALS: kava-kava, valerian, chamomille, grapefruit juice, taking after a meal
Nursing Interventions Use cautiously in pts w/ hepatic/renal impairment. Have safety protocol in case of adverse CNS reactions. It does not prevent withdrawal symptoms, so monitor pts carefully who are being tapered off of benzos. or other hypnotic sedatives.
Client Education Evaluation of Medication Effectiveness An increase in the sense of well-being. Improve sexual desire in women.
ACTIVE LEARNING TEMPLATES
Advise to take consistently w/ or w/o food. Caution against consuming large amounts of grapefruit juice. Inform that it takes 1+ wks to see effects of the drug. Do not attempt activities that require alertness, until effects of therapy are known. DO NOT double dose. Advise against drinking or consuming other CNS depressants while taking. Emphasize the importance of follow-up exams. Notify provider if chronic abnormal movements occur or if pregnancy is expected.
