PURPOSE OF MEDICATION
Expected Pharmacological Action It binds to bacterial cell wall which results in cell death
Therapeutic Use Bactericidal action against susceptible organisms Used to treat patients with severe infections MRSA Infections in patients with an allergy to penicillin
Complications Otoxocity, hypotension, nausea, vomiting, nephrotoxicity, rashes, leukopenia, phlebitis, back and neck pain
Medication Administration IV and by mouth [PO] PO for colitis Give IV for all other infections. -Administer slowly and follow recommendations for dilution -Infuse separately, if possible (incompatible in solution with many other IV drugs)
Contraindications/Precautions Hypersensitivity Renal insufficiency (reduce dosage) Impaired hearing Colitis

Nursing Interventions Assess for infection Obtain specimen for culture and sensitivity before initiating therapy. Monitor IV site closely Monitor BP throughout IV fusion May cause additive otoxicity and nephrotocity with other otoxic and nephrotoxic drugs May enhance neuromuscular blockade fron nondepolarizing neuromusucular blocking agents
Metformin may cause an increased risk for lactic acidosis Drugs that are toxis to hearing or kidney increase the risk for ototoxicity or nephrocity Incompatible in a solution with multiple other IV drugs Client Education Evaluation of Medication Effectiveness Resolution of signs and symptoms of infection Length of time for complete resolution depends on organism and site of infection
