Client Identifiers The client, Zachary Michaels, is a 20-year-old Caucasian male. He is currently single and attending college as a sophomore. Presenting problem Mr. Michaels professor convinced him to come into the Student Health Services office. He was concerned about his apparent apathy towards both class attendance and conversation. Mr. Michaels roommate echoes the professor’s concerns, stating that Zachary “sometimes spends days never leaving his room and only has minimal conversation,” Problem History Mr. Michael's, to our current knowledge, has only been showing the severity of his current symptoms for the past semester. He has obviously been able to keep his grades and class attendance up to university standards in order to reach his sophomore year. He claims to have always been a “loner,” which shows antisocial tendencies but little else. Psychiatric history No psychiatric history is currently available on the client. Medical History No medical history is currently available on the client. Substance History No substance history is currently available on the client. Developmental History The only developmental history provided by the client is his family of origin being separated. His parents divorced and he was spilt from both his mother and sisters. Education/Occupation Mr. Michaels is a college sophomore with no outside income responsibilities. Income/Living situation The client lives in student housing, a dormitory, with a roommate. According to the available information he has no outside source of income. Family of Origin/social supports The client’s family of origin is separated, which he cares little about. He only stays connected with his father, whom he lived with prior to college, through email and texting. He has little to no contact with either his mother or two sisters. Legal History

No legal history for the client is available currently. Mental status check The client walks in with a “shuffle” and “unkempt” hair, displaying psychomotor issues. The client also presents a very apatetic affect, showing little to no emotion on his face. Mr. Michaels remains very restrictive in his commination throughout our session. He often seems distracted, uninterested, and at times hostile. When he does communicate, however, it is often confusing. He speaks in idiomatic language and often veers off topic. Preliminary Diagnosis Schizotypal Personality Disorder (Premorbid ) 301.22 (F21) The client shows both delusions and a reduced compacity for social connections. Throughout the interview he is both self-absorbed and emotionless. Rationale Schizotypal Personality Disorder was diagnosed over Schizoid Personality Disorder due to the clients’ delusions and how they mirror schizophrenic episodes. By diagnosing the schizotypal rather than schizoid both his delusions and asocial behavior patterns can be accounted for. Diagnostic criteria A is met by the client’s interpersonal relationships. He calls his nuclear family “idiots” and barely speaks to them, labels himself a “loner”, and only speaks to those in his gaming community who are “intelligent enough” to understand him. He also shows little to no affect during conversation. He shows perceptual deficits by maintaining his delusion about his gaming community, “Third Flight”, being a crucial militant tool. When speaking about this he becomes increasingly paranoid and metaphorical. Criteria B is met by the clients lack of prior diagnoses of schizophrenia or bipolar disorder. The Premorbid specifier is listed due to the lack of diagnosis of schizophrenia. Psycho-diagnostic test(s) The only current testing for this disorder is the adherence to symptom pattern as listed in the DSM , however, by utilizing the Schizotypal Personality Questionnaire, the disorders severity can be noted. Diversity The client is a middle class, white, adolescent. Diversity has minimal affect. Strengths Mr. Michaels, while apathetic, does have professors who care for him. This will aid in his ability to seek out and retrieve treatment.

Limitations The marked social patterns of this disorder make treatment difficult. The patients are quite often in denial of treatment need and the presence of disorder....
