To communicate with other care team members and to help plan and evaluate residents care nurses must use 5 steps: o Assessment: getting information from many sources, including medical history, physical assessment and environment ▪ What is the resident's status including health and environment? ▪ Purpose is to identify actual or potential problems o Diagnosis: ▪ Identifying health problems after looking at all residents needs o Planning: ▪ Setting goals and creating a care plan in accordance with residents preferences to meet residents needs • What are the goals (expected outcomes) of providing care
