Likely Medication/Food Interactions: Iron salts: May decrease absorption of ciprofloxacin reducing antiinfective response. Give at least 2 hours apart. Steroids: May increase risk of tendinitis and tendon rupture. (Black Box Warning) Tizanidine: Increases tizanidine levels, causing low BP, somnolence,
dizziness, and slowed psychomotor skills. Use together is discouraged. Yerba Mate: May decrease clearance of herb’s methylxanthines and cause toxicity. Discourage use together.
Administration/Interventions/Evaluation of Effectiveness/Laboratory Work
Black Box Warning: Fluoroquinolones have been associated with disabling and potentially irreversible serious adverse reactions that have occurred together, including tendinitis and tendon rupture, peripheral neuropathy, and CNS effects. Drug is associated with increased risk of serious adverse CNS reactions.
May increase alkaline, phosphatase, ALT, AST, bilirubin, BUN, creatinine, LDH, and GGT levels.
May decrease WBC, neutrophil, and platelet counts.
Contraindicated in hypersensitivity to fluoroquinolones.
Patient Education: What will the safe nurse teach the client to DO or expect or report? Tell patient to take drug as prescribed, even after feeling better. Advise patient to drink plenty of fluids to reduce risk of urine crystals. Advise patient not to crush, split, or chew the extended-release tablets. Tell patient that tendon rupture can occur with drug and to notify prescriber if pain or inflammation occurs.
