Description

Standardized Cognitive Screening Instruments Assessment

Stated Purpose Quick measure of the severity of cognitive dysfunction. Multiple versions allow for tracking recovery.

Intended Population Elderly persons with dementia or clients with confusion post TBI

Area(s) Evaluated Orientation (Time), concentration, and recall

Brief and efficient measure of cognitive, linguistic, and communicative abilities of clients with TBI. Can track recovery patterns due to sensitivity to small performance changes.

Acute and long-term head injured adults between Rancho Los Amigos Levels of Cognitive Functioning III and VIII

Cognitive Assessment of Minnesota [CAM] (Rustad, DeGroot, Jungkunz, Freeberg, Borowick, & Wanttie, 1993)

Screens a wide range of cognitive skills hierarchically graded from simple to complex emphasizing function. 17 subtests arranged to test the fund of knowledge, manipulation of knowledge, social awareness and judgment, and abstract thinking.

Adults with neurologic impairments. Normed on ages 18-70.

Cognitive Competency Test [CCT] (Wang, Ennis, & Copland, 1992)

Multidimensional measure of mental competency using simulated daily living situations requiring cognitive skills for safe and independent living.

Normed on adults ages 50 – 93 and validated with clients with stroke and dementia

Orientation, attention, following commands, linguistic organization, reading comprehension, naming, memory, and visualspatial skills Attention span, memory, orientation, mathematics, visual neglect, following directions, object identification, judgment, reasoning, and safety Memory, management of money, care of clothing / household, orientation, calculation, problem solving / judgment, comprehension / understanding, social skills, social competency, safety, and mental fitness

Galveston Orientation and Amnesia Test [GOAT] (Levin, O’Donnell, & Grossman, 1979) Lowenstein Occupational Therapy Cognitive Assessment [LOTCA] (Katz, Itzkopvich, Elazar, Averbuch, & Rahmani, 1989)

Orientation to person, place and time, and recall of events before (retrograde amnesia) and after (anterograde amnesia) the injury. Standardized, reliable, and valid battery provides baseline data to evaluate cognitive – perceptual abilities and disabilities and assists in developing and evaluating treatment goals.

Adult clients post coma

Orientation and long-term memory

LOTCA designed for clients ages 6 - adults with brain-injury LOTCA-ch for children (Katz & Parush, 2001) LOTCA-G geriatric clients with brain injury (Elazer, Itzkovich, & Katz, 1996)

Orientation, visual and spatial perception, visual motor organization, thinking operations. Attention and concentration are observed for all subtests

Blessed Dementia Rating Scale (Blessed, Tomlinson, & Roth, 1968) Brief Test of Head Injury (BTHI) ( HelmEstabrooks & Hotz, 1991)

Assessment Middlesex Elderly Assessment of Mental State [MEAMS] (Golding, 1989) Mini Mental State Exam [MMSE] (Folstein, Folstein, & McHugh, 1975)

Modified Mini-Mental Examination [3MS] (Teng & Chui, 1987)

COGNISTAT [Neurobehavioral Cognitive Status Screening Examination] (Kiernan, Mueller, & Langston, 1983)

Repeatable Battery for the Assessment of Neuropsychological Status [RBANS™] (Randolph, 1998)

Stated Purpose Screening test to detect gross impairment of specific cognitive skills in the elderly. Differentiate between functional illnesses and organically-based cognitive impairments. Two parallel versions allow for re-test. Can be used to screen for cognitive impairment, estimate the severity of impairment, track cognitive changes, and document response to treatment. Modification of the original MMSE that includes an expanded number of items within each section and four new test items. Scores are expanded from 0 -100 thus increasing the sensitivity of the screening. Brief assessment tool to screen for cognitive dysfunction. Provides profile scores for each of the domains assessed. Initial test item in each domain is at a near-normal level of difficulty with subsequent items provided only if initial item is failed. Brief, repeatable measure of cognitive decline or stability over time. Capable of testing adult with normal functioning through moderately severe dementia. Portable for easy bedside administration

Intended Population Adults ages 65 and older

Clients with moderate to severe cognitive impairments. Valid and reliable in psychiatric, neurologic, geriatric, and other adult medical populations Clients with moderate to severe cognitive impairments. Adults with suspected cognitive impairments. Norms available on ages 20 – 92. Validated on neurosurgical patients with confirmed lesions. Adults ages 20 to 89 years with neurologic injury or disease such as dementia, head injury, and stroke

Area(s) Evaluated Orientation, memory, new learning, naming, comprehension, arithmetic, visio-spatial skills, perception Orientation, attention, calculation, recall, language, praxis and design copying. Items of original MMSE plus semantic fluency, delayed memory, remote personal information and abstraction. Language constructions, memory, calculations, and reasoning. Attention, level of consciousness and orientation are independently scored. Attention, immediate memory, delayed memory, visuospatial / construction, and language
