Clint education: THR Introduction+ explain role: Hi I am shivani, I am an occupational therapist. How are you today Mr/Mrs…….. I know we met before, but you probable meeting many other health professionals along the way. So do you remember my role as occupational therapist? Response….. So as occupational therapist I will enable you to engage in the self-care activities that are important and meaningful for you. So I got referral from nursing staff to see how you been going with your dressing.  After you hip replacement did you carry out dressing task by yourself? So how have you been dressing since you been in hospital?  Impacting on your lower body dressing skills- no bending. Also, hip replacement impact on your strength to move or transfer on/off and range of motion.  Did you experienced difficulty before coming into the hospital? Is there anything in particular you concerned about dressing? Purpose: The focus of today’s session would on helping you start putting on socks independently, with using some assistive equipment. This session will take about 10-15 minutes. Will that be ok with you?

Hip precaution: Before getting in procedure of wearing sock, could you please tell me some hip precaution? Alright, I will go through and show you how will be helpful in dressing especially socks. HIP PRECAUTION: Not to flex hip, means no lifting or bending hip beyond 90 degree. Do not do twisting. Do not crossing your leg at all time (sitting). Not turning your feet in, cause rotate hip.  Looking at position of sitting – at home set to wear socks in chair Are you happy to start with socks today? Is there anything you intend on wearing?

Sock aid- benefit of sock aids it prevent you from bending forward to wear socks and it adheres the hip precaution to protect your new hip. It’s easy and painless. Also will show other equipment which might help you along the way.  Would you like me to show you whole things first or would you to prefer me to talk you through? To put on socks or hose 1. Place the sock aid into your sock or stocking. Make sure the heel of your sock is at the back of the sock aid. For support socks, be sure to spread the sock over the sock aid evenly, without “bunching.” 2. Hold the sock aid by the straps with both hands. First do the surgical leg. While holding the straps, drop the sock aid to the floor in front of the foot on your weaker leg. 3. Slip your foot into the sock aid. Then pull on the straps to pull the sock aid onto your foot (see figure to right). 4. Pull until the sock is up your leg. Keep pulling until the sock aid comes out of your sock. 5. Follow the same steps to put a sock on the other foot. Follow this procedure with surgical leg first, then non-surgical To remove socks or hose 1. Hold your long-handle shoe horn or pick up stick. Use it to push the sock off your foot. Slide or push down along the back of your leg and heel. 2. Use your Reacher to pick up your socks from the floor. Follow this procedure take off sock from non-surgical leg, then surgical leg

Feedback: how do you feel now, where you comfortable wearing socks and shoes with the equipment? Response. Any point you experienced difficulty in wearing sock or shoes? Response…… This will take lots of practice, don’t feel upset, and if you don’t achieve mastery at first go. Have few practice and see how you find it. Recommendation: just recommendation is try to follow the hip precaution when you do dressing or any other self-care activities. Also, need to follow hip precaution for at least 6-8 weeks after your discharge.  Is there any question or any concern? Response – types of socks you wear Conclude: So, I hope it’s gave you an idea of how to use sock aid. Evaluate:  Specific needs to know why, what and how of learning. I explained the purpose of session and procedure. This allowed client to be more committed and engage in the new learning.  Self-directed approach to learning, I make sure I ask client for their consent to proceed and question or concerns they might have after the session and help to set relevant goal to manage wearing socks through sock aid.  Accessing client existing knowledge through asking client to recall the hip precaution and making sure client remembered and was filling gaps when needed.  Problem centred approach to learning, I allowed client to have practical on using assistive equipment by providing supportive learning environment.  Client readiness to learn impact on outcome by clarifying with the client the impact of THR on the self-care activities and then implement techniques to overcome them. Through building chain of behaviour used to teach putting on sock aid.  Motivation to learn impacts on learning by showing visual cues and modelling to make learning easier to achieve their goal. Also provide feedback by asking how they felt after using equipment, did they had any difficulties. This all principal I undertook with my client in the session. To make the learning easier and support client centred approach to learning....
