CoSHH: Control of Substances Hazardous to Health By completing this document you are assessing the risk involved in handling, use and disposal of the named substance, and by signing it YOU BECOME RESPONSIBLE for its use. Numbers by each section relate to those in the Safety Data Sheets (SDS). 1) FULL NAME OF CHEMICAL with chemical formula, including ‘common’ name, and grade where possible. (examples: Sodium Hydroxide (NaOH) GPR; Methanol ‘HPLC grade’; Boric Acid ‘ANALAR’) Voges Proskauer test reagent: alpha naphthol (C10H7OH) 5% in 10% sodium hydroxide (NaOH)

PURPOSE FOR WHICH YOU NEED THE SUBSTANCE: (brief description of what you are going to do with it and why) Voges Proskauer test

QUANTITY /AMOUNT OF SUBSTANCE REQUIRED: (examples: 5g; 250ml; 1000 units; 2 slopes) 10 ml in dropper bottle

QUANTITY /AMOUNT OF SUBSTANCE TO BE PREPARED from original above: (examples: 500ml 0.5M solution; 20 petri dishes) Be aware: if what you make is radically different from the original substance A SEPARATE CoSHH FORM WILL NEED TO BE PREPARED. use one drop per test

2) IDENTIFIED HAZARDS (EXPOSURE RISK): SEE ALSO BOX “11” (Toxicological Info) in SDS (examples: causes burns; flammable; toxic. Is it harmful to the environment? ie: do not put silver stain in the drains! Harmful in contact with skin and if swallowed. Irritating to respiratory system and skin. Can cause burns to skin. Risk of serious damage to eyes. Students must wear PPE and gloves.

3)WHAT IS IT? (examples: acid; alkali; solvent; inorganic salt) organic compound in 10% alkali

4) FIRST-AID MEASURES: (What do I do if it gets on me or someone else?) If inhaled If breathed in, move person into fresh air. If not breathing give artificial respiration Consult a physician. In case of skin contact Wash off with soap and plenty of water. Consult a physician. In case of eye contact Rinse thoroughly with plenty of water for at least 15 minutes and consult a physician. If swallowed Do NOT induce vomiting. Never give anything by mouth to an unconscious person. Rinse mouth with water. 5) FIRE-FIGHTING MEASURES: (What do I do, and with what, and where is it kept?) Inform staff Use water spray, alcohol-resistant foam, dry chemical or carbon dioxide. Special hazards arising from the substance or mixture Carbon oxides, Sodium oxides

6) SPILLAGE/ACCIDENTAL RELEASE (If I spill it, what do I immediately do? In addition: always tell those around you. INFORM THE TECHNICAL STAFF) Inform staff: Use personal protective equipment. Avoid breathing vapours, mist or gas. Ensure adequate ventilation. Prevent further leakage or spillage if safe to do so. Do not let product enter drains. Discharge into the environment must be avoided. Soak up with inert absorbent material and dispose of as hazardous waste. Keep in suitable, closed containers for disposal. 7) HANDLING and STORAGE: (Are there any special instructions to obey? examples: “NEVER ADD WATER TO THIS MATERIAL”; “Avoid contact with skin and eyes”; Do not store with ***”) Avoid contact with skin and eyes

ALL ORIGINAL CHEMICALS, SOLVENTS AND DILUTED CHEMICALS CONTAINING SOLVENTS MUST BE STORED IN THE APPROPRIATE PLACE IN PREPROOMS ONLY UNLESS TOLD OTHERWISE. DILUTED CHEMICALS MAY BE STORED IN STUDENT CUPBOARDS IN APPRPROPRIATE CONTAINERS.

8) PERSONAL PROTECTION: ALL USERS MUST UTILISE A MINIMUM OF FASTENED LABCOAT AND SAFETY SPECS UNLESS ADDITIONAL PROTECTION IS SPECIFIED. Tick as specified by SDS: Disposable gloves Heavy rubber gloves Disposable apron Face Shield Extraction/Flow hood Safety Cabinet Other If other give details: 9) PHYSICAL PROPERTIES: (examples: Is it soluble in water? If not, in what? Is it flammable? Does it have to be stored in special containers, or in the dark? Any other important considerations?) water soluble

13) DISPOSAL CONSIDERATIONS: (How do I get rid anything I use or make?) DO NOT DISPOSE OF CHEMICALS DOWN THE SINKS UNLESS GIVEN PERMISSION!! If you are in doubt, ASK THE TECHNICIANS. Product Offer surplus and non-recyclable solutions to a licensed disposal company. Dissolve or mix the material with a combustible solvent and burn in a chemical incinerator equipped with an afterburner and scrubber. Contaminated packaging Dispose of as unused product.

ASSESSED RISK: ie, are there any risks involved in using this substance? Low. Only one drop of dilute solution per culture, Use in fume cupboard, PPE and gloves must be worn.
