THE LAST WILL AND TESTAMENT This is the LAST WILL AND TESTAMENT of me, XXX (NRIC NO: XXX of 1866, Jalan Dutamas Raya, 51200 Kuala Lumpur, Malaysia. 1) I hereby revoke all my former wills and all other testamentary dispositions hereto before made by me and declare this to be my LAST WILL AND TESTAMENT.

2) I hereby appoint the following persons as my Executors/Trustees :a) My cousin; XXX (NRIC NO: XXX) of No.1, Jalan BU4/2, Bandar Utama, 47800 Petaling Jaya, Selangor; and b) My friend; XXX (NRIC NO: XXX) of 12/1, Taman Mas, 84400 Sg Mati, Ledang, Johor, Malaysia. (hereinafter referred to as “my Executor/Trustees”).

3) If her/they predecease me of are unable or unwilling to fulfill this role, I appoint the following person as my Executor/Trustee:a) My cousin; XXX (NRIC NO: XXX) of 7-1, Jalan Bumi, Taman Setia, 17500 Tanah Merah, Kelantan, Malaysia.

4) I declare that in the interpretation of this Will the expressions “my Executor/Trustees” shall where the context permits mean and include the executors and trustees for the time being hereof whether original or substituted.

5) I direct my Executor/Trustees to pay all my just debts, testamentary, funeral, executorship out of my estate.

7) My only sibling is SISTER (NRIC NO: XXX) of PT 88, Taman Bahagia, 43000 Kajang, Selangor, Malaysia. (hereinafter referred to as “SISTER”). 8) I am married to HUSBAND (NRIC NO: XXX) of 1866, Jalan Dutamas Raya, 51200 Kuala Lumpur, Malaysia. (hereinafter referred to as “HUSBAND”).

9) I have number of child(ren) living children, named: I. SON (NRIC NO: XXX) and II. DAUGHTER (NRIC NO: XXX) of 1866, Jalan Dutamas Raya, 51200 Kuala Lumpur, Malaysia. (hereinafter referred to as “SON” and “DAUGHTER”).

I GIVE, DEVISE AND BEQUEATH all the monies standing to my credit in my bank account with Adventure Bank Bhd Kuala Lumpur Branch, Account no. 002-X-12345 to SISTER.

I GIVE, DEVISE AND BEQUEATH all the monies standing to my credit in my bank account with Okay Bank Bhd Kota Bharu Branch, Account no. O-32104567 to HUSBAND.
