Chytraus 3 Intake Memo: Client: Jane Doe Date: 11 February, 2019. I interviewed potential client Jane Doe about her car accident that occurred on January 1, 2018 at 2:00 a.m. She had been driving her own SUV, which is insured by All State, with two passengers in the vehicle when the accident occurred. Neither passenger was injured. There was no bad weather or traffic patterns to cause the accident as well as it happened in a well-lit area. Jane was not at fault for the accident and there are multiple wheel witnesses who can confirm this. Although Jane did not retain any of the witnesses contact information, there is a police report that states their names. The police report also includes photos of the accident. Neither driver made a statement at the time, spoke with each other at the time of the accident or claimed fault. Jane has since reported the accident to her insurance and her car’s damage was estimated to be about $2,500.00. Jane was in good health before the accident. Jane has never been in an accident before and has suffered significant injuries, including multiple surgeries on her fractured wrist and forearm, extensive physical therapy to regain functional use of her hand, which is permanently injured, as well as on going neck and back pain. Jane is a 35 year old woman who is married and has a three year old son. Before the accident she was working as a surgeon, but because of the extensive damage to her hand, she is unable to return to work. Her planned age of retirement was 63 and has a salary of $200,000 annually, so it is estimated she will be losing $560,000.00 of income due to this injury as well as she has experienced about $15,000.00 in medical bills....
