PURPOSE OF MEDICATION

Expected Pharmacological Action Binds with cytokine tumor necrosis factor alpha, preventing it from binding with its receptors. Infiltration of inflammatory cells into inflamed intestines and joints declines.

Therapeutic Use To control moderate to severe Crohn’s disease long-term

Complications CNS-> Dizziness, chills, fatigue, fever, headache, meningitis CV-> Arrythmias, bradycardia, chest pain, edema GI-> Abdominal hernia, abdominal pain, hepatic failure

Contraindications/Precautions Patients that are breastfeeding are contraindicated for this medication

Interactions Live vaccines-> increased risk of adverse vaccine effects Abatacept, anakinra, etanercept, tocilizumab-> increased risk of neutropenia and serious infections.

Medication Administration IV->5mg/kg infused over 2hrs, repeated 2 and 6 weeks after first infusion

Nursing Interventions Know that infliximab therapy shouldn’t be started in a patient with an active infection. Use cautiously in elderly patients because they have a higher risk of infection. Monitor patient’s CBC

Client Education Evaluation of Medication Effectiveness Decreased pain and swelling with decreased rate of joint destruction and improved physical function in patients with ankylosing spondylitis psoriatic or rheumatoid arthritis.

Teach patient that med will take effect in 1 to 2 weeks. Urge patient to report evidence of infection, such as cough, painful urination, and sore throat. Infusion reaction can happen for up to 12 days.
