Learning Theory with rationale: To educate client about the medication they have been prescribed by their physician (in this case Warfarin 5 mg, PO once daily), to ensure the medication is being administered safely and effectively. During the teaching process, the nurse and client will communicate information, emotion, perception, and attitude to each other (Kozier et al.,2018, p.502). Client education is a major aspect of nursing, to ensure client’s safety and to make sure the specific medication follows a therapeutic effect. Nurse’s role is to promote healthy lifestyle, learning theories through the application of health knowledge, while educating clients and their families Teaching Client C how to administer his/her medication safely. Educate the client about the effects, adverse effects, and precautions of the medication. Teaching will be done in a safe and private environment, through verbal communication, in a one on one setting since client is alert and oriented. Teaching should begin at the time of diagnosis or admission. Prior to teaching, nurse is to assess the client’s learning style, religion beliefs, level of understanding, reading level, and their support system (Kozier et al.,2018, p.507). Patient will list the therapeutic and adverse effect of the medication, dosage routes, and methods of adequate selfadministration, drug interactions, and any other special considerations (Jarvis, 2019). CONTENT TIME Warfarin (Coumadin) It’s an anti-coagulant medication that is used to prevent or manage platelet plugs by blocking vitamin K, which will result in regulating INR.
