Scenario

Week 25
You are the GP practice pharmacist undertaking a targeted medication review for patients with type 2 diabetes and HbA1c > 7.5%. The consultation will take place remotely via video call due to COVID-19 restrictions. Your next patient – Mrs Denise Farringway, aged 74 years – is not able to participate in the consultation due to severe cognitive impairment caused by her vascular dementia, but her main carer – Ms Yvonne Carter – is instead available to help answer your questions.
Task
The role play should begin as such that your outgoing video call has just been accepted by the carer on their device. You are expected to take a detailed history, identify key therapeutic problems, and suggest a reasonable course of action and/or offer suitable advice.
Assumptions
The GP practice pharmacist is qualified as an independent prescriber – you are permitted to change doses, strengths and formulations of drugs; but not able to start, stop or switch drugs to an alternative without input from the GP or specialist. You are not required to recommend or prescribe any specific drugs or products in this OSCE.
