Pharm Phorm Week 1 Prototype drug: Ativan (Lorazepam) Class: Benzodiazepines Clinical Indications: (top three) 1. Preoperative Sedation 2. Anxiety-Disorder 3. Antiemetic prior to chemotherapy Mechanism of Action: Depresses the CNS, probably by potentiating GABA, an inhibitory neurotransmitter Side/Adverse Effects: (adverse in bold) 

Drowsiness



Dizziness



Constipation



Heartburn

● Rashes ● Lethargy ● Respiratory depression, cardiac arrest, blurred vision. Route and Dosage: Adults only ● PO: Anxiety: 1-3 mg 2-3 times daily (only up to 10 mg/daily) Insomnia: 2-4 mg at bedtime ● IM: Preoperative sedation: 50mcg/kg 2hr before surgery (not over 4mg) ● IV: 44mcg/kg

Nursing Implications: (top 3) 1. Assess location, duration, characteristics, and frequency of seizures. (Precautions should be initiated) 2. Prolonged high doses can cause physical or psychological dependence.

3. Assess the client’s anxiety and mental status for degree of manifestations prior to and periodically throughout therapy.

Patient Teaching ● DO NOT skip or double up on doses. ● DO NOT stop abruptly, could causes withdraws. ● Avoid driving or operating machinery when taking this medication. ● Should only be used short-term.

Other Considerations: (controlled substance; narrow therapeutic window; short half-life; peak and trough; antidote, etc.) ● Category D for pregnancy ● PO: onset: 15-60 min, peak: 1-6 hours, duration: 8-12 hours ● IM: onset: 30-60 min, peak 1-2 hours, duration: 8-12 hours ● IV: onset 15-30 min, peak 1-2 hours, duration: 8-12 hours ● Antidote to overdose: Romazicon (flumazenil) IV
