● Arrhythmias Route and Dosage: Adults only PO: 2.5 – 10 mg/day divided Q6 – 8hr (should not exceed 40mg/day) IM: 12.5 – 25 mg/day (should not exceed 100mg/injection) Nursing Implications: (top 3) 1.Assess mental status routinely 2. Patient Teaching ● Medication should be taken as prescribed; if a dose is missed, miss dose should be taken within the 1hr or skipped and take next miss. (DO NOT double up on dose) ● Do not stop medication abruptly to avoid withdrawal symptoms. ● Clients should notify HCP if sore throat, fever, unusual bleeding or bruising, rash, weakness, tremors, visual disturbances, dark-colored urine, or clay-colored stool occur.
Other Considerations: (controlled substance; narrow therapeutic window; short halflife; peak and trough; antidote, etc.) ● Monitor the hepatic and renal function of the client ● Avoid alcohol and CNS depressant usage
