2 PICOT- Nursing Burnout Background Nursing burnout affects all aspects of health including mental, physical, and emotional exhaustion due to work stressors. Work stressors include long shifts, increasing workload, highstress work environments, caring for multiple patients, and making quick treatment decisions. Nurses working night shifts have shown to develop mental health issues as a result of the timeframe of their shifts, whereas nurses working day shifts have not shown mental health issues as a result of the timeframe of their shifts. The mental health issues that nurses develop as a result of working night shifts can affect them negatively in the long-term. Significance in Nursing Night shift nurses often have an altered circadian rhythm, sleep-disturbance, and “shift work disorder” (Ferri et al, 2016). Shift work disorder is often identified by various symptoms including alteration of circadian rhythm, insomnia, fatigue, and increased day sleepiness. An altered circadian rhythm can lead to sleep disorders causing nurses to have difficulty falling asleep, interrupted sleep, and insomnia. Night shift nurses in particular have poor alertness and job performance due to sleep deprivation and poor circadian rhythm. Nurses that have a decrease in alertness time often have a poor reaction time to patients leading to medical errors. Nurses who work night shifts often experience mental health problems such as psychological distress, anxiety, and paranoid disorders. Paranoid disorders can cause nurses to have fears and worries that are unreasonable, as well as causing them to act irritational. In comparison to night shift nurses, day shift nurses often have an increase of social support (Khalid & Sarwar, 2015). Increase of social support positively influences day shift workers work motivation and job performance. Whereas night shift nurses have a decrease social support from their peers and coworkers causing a lack of work motivation. In a study conducted

3 PICOT- Nursing Burnout by Ferri et al (2016), “day shift nurses are often less likely to be associated with risk factors of predisposing health conditions and have an increased job satisfaction.” However, working longterm night shifts are often associated with increased risks of predisposing conditions such as cardiovascular diseases, gastrointestinal diseases, fatigue, altered circadian rhythm, and sleep disturbance (Ulas et al., 2012). However, day shift and night shift nurses both experience obsessive-compulsive disorder, also known as OCD. OCD can cause nurses to repeatedly hand wash and cause nurses to be paranoid about mistakes. Oxidative stress occurs when there is a disturbed balance of free radicals and antioxidant, causing damage that can lead to numerous diseases. Ulas et al. (2012), “states that both night and day shift nurses experience an increase of oxidative stress due to an increased workload without enough time to rest in between.” Nurses are often not given enough breaks in order for them to rest and refuel during their shifts, this can lead them to be stressed and paranoid about their workload. PICOT Questions - In nurses, how does working night shifts as compared to working day shifts influence nurses’ health over a five-year period? - In nurses, how does working night shifts as compared to working day shifts influence nurses’ alertness over their patients’ healthcare over a a month period? - In nurses, how does the mental health of nurses working night shift as compared to nurses working day shift influence their daily lives? - In NICU patients, how does the effects of nurses who are experiencing burnout as compared to nurses who are not experiencing burnout influence the patient’s treatment until discharge day?
