Description

Practical Nursing Diploma Program Pre-Clinical Nursing Care Research Assignment “Prep and Plan” Medical-Surgical Diagnoses:

Dx: Patient A.P, 12-year-old Hx: Asthma in moderate respiratory distress

Anticipated assessment data for this patient: 1. Chest pain 4. Coughing 2. SOB 5. ______________________________ 3. Exercise induced 6. ______________________________ List 2 priority “NANDA” Nursing Diagnoses based on the above assessment data: 1. Ineffective breathing pattern due to swelling and spasm of the bronchial tubes in response to inhaled objects related to asthma 2. Activity intolerance d/t feeling fatigue from asthmatic attacks

Identify specific nursing interventions to support these priority nursing diagnoses: (action plan) 1. Assess resp rate, depth and rhythm 4. Assess presence of weakness 2. Assess breath sounds 5. allow time to rest 3. Assess for signs of dyspnea 6. Modified activity until asthma controlled

Based on your prep & plan what will you pay special attention to: (What do you see as the patient’s priority issue?) As a nurse I will pay special attention to signs of dyspnea shortness of breath chest pain and any signs of stress. The main priority is making sure there is a Ventolin inhaler is in reach at whole times.

What teaching do you think your patient will require? Teach the child and parent signs of dyspnea. have the child sit down if feeling out of breath or experiencing chest pain. teach child and parent how to use an aero chamber with an inhaler. Teach the child to bring Ventolin inhaler + aero chamber everywhere they go in case of acute asthmatic attack.
