Assess the client’s vital signs as needed while in distress 2. Assess the respiratory rate, depth and rhythm 3. Assess breath sounds and adventitious sounds (wheezes and stridor) 4. Assess the presence of weakness and fatigue caused by airway problem 5. Encourage activities such as quiet play, reading, watching movies, games during rest 6. Schedule and provide rest periods in a calm peaceful environment

Based on your prep & plan what will you pay special attention to: (What do you see as the patient’s priority issue?)  I see ineffective breathing pattern and risk for activity intolerance as my patient’s priority issues. I will pay close attention to my patient’s vital signs while checking for dyspnea,

coughing and use of accessory muscles. Along with checking my patient for any adventitious sounds (wheezes and stridor)

What teaching do you think your patient will require? 

I think my patient will require teaching on how to properly use their inhaler and monitor their own breathing patterns. My patient needs encouragement to rest or doing calming actives while resting. Also my patient will require help setting up a schedule for rest periods to help with their asthma.
