Bulk-forming fiber laxative; GI agent; Herbs and natural products CATEGORY CLASS ______________________________________________________________________ PURPOSE OF MEDICATION

Expected Pharmacological Action Composition of dietary fiber and mucilage, when mixed with water, is capable of absorbing up to 40x its own weight in water, forming a gel-like mass. This gel-like mass subsequently moves down a pt's digestive system and makes stool softer by increasing its water content. It simultaneously lubricates the intestine, and the increase of mass of the stool stimulates the bowel wall to begin peristalsis.

Therapeutic Use To treat occasional constipation, diarrhea, or bowel irregularity; To help lower cholesterol/blood sugar adjunct to diet; To help reduce hemorrhoid pain.

Complications Abdominal or stomach cramping, flatulence, gas, bloating

Medication Administration Powder, granules, capsule, liquid, or chew wafer - usually mixed with or taken with a full glass of water.

Contraindications/Precautions Allergy to psyllium, bowel obstruction or spasm, difficulty swallowing, esophageal stricture, kidney disease. May lower blood sugar, putting pt at risk of hypoglycemia. May have to hold DM medications.

Interactions Digoxin, Salicylates, Nitrofurantoin, Amitriptyline, Doxepin, Imipramine, Carbamazepine, Lithium, Amobarbital, Amlodipine, Amiodarine, Amiloride, all diabetic medications. Talk to prescriber before taking any diabetic medications, as Psyllium may increase risk of hypoglycemia.

Evaluation of Medication Effectiveness Assess pt for therapeutic effect. Assess pt for abdominal distention, bowel sounds. Desired outcome is usually a soft, formed bowel movement.

ACTIVE LEARNING TEMPLATES

Nursing Interventions Give with 8 oz of water and if tolerated 6-8 glasses of water/day. Do not give simultaneously with other meds. Monitor pt for s/s of GI distress. Monitor for s/s of adverse effects.

Client Education Take 1-4 hrs before other meds to avoid a reduction/delay in absorption of other meds. Always take with full glass of water and drink 6-8 glasses of water/day. Educate pt on healthy habits for normal BM such as adequate water intake, exercise, and adequate fiber intake.
