Description: While doing placements in aged care facility, I noticed that one of the carer did not performed hand hygiene while feeding one of the resident. I wanted to ask her about hand washing but did not asked, as she was very senior carer and I was a student. Feelings: Initially, I was very hesitant and felt intimidated to ask her about not doing hand washing prior to feeding the resident. I felt regret for not intervening the carer to wash hands before the procedure. Evaluation: This small negligence has both positive and negative aspects. The positive aspect is that the carer realised her mistake and responded very positively when asked about not washing hands. On the negative side, lack of effective and frequent hand washing might result in infection (Friedrich, Kappler & Mosler, 2018). Analysis: On analysis I realised that hand washing is one of the basic and very effective means of controlling infection. In fact it is very crucial to wash hands to prevent contamination before and after any procedure (Centres for Disease Control and Prevention, 2019). Conclusion: From this incidence, I learnt that I should be more proactive and vigilant when it comes to patient safety because patient safety is one of the main priorities of health care workers (Nursing and Midwifery Board of Australia, 2019). Action plan:

In future, my aim is to be active and assertive when providing care to the residents in order to promote their safety. If I encounter such situation again in my life, I will be quicker to take action for patient benefit.
