Time Frame- 2 quarters

1. By the end of the 1st half quarter instruction when given the Driver’s Education Manual Dan will improve his reading skills from 0 (zero) success at passing the test, by the first half of the first quarter Dan will pass the drivers permit and driver’s license improving his reading skills.

1. Dan’s family can help him study by practicing definitions in the driver’s manual. 2. Dan can make a graphic organizer to help with remember what he read in his driver’s manual. 3. His family can practice giving Dan questions.

2. By the end of the 2nd quarter, Dan will improve his writing skills from 90% on 1- page form to 100% on multiple page forms by the end of the 2nd quarter.

Timeframe- by the end of 4th quarter. Condition- read a story at the 4th grade level. Behavior- Dan will respond to short answer questions with complete sentences. Criterion- respond to short answer questions with complete sentences scoring 80% to 90% accuracy.

1. By the end of 4th quarter, after reading a story at the 4th grade level, Dan will respond to 4 out 5 short answer questions at 65% to a 75% accuracy with minimal mechanical errors. 2.

By the end of 4th quarter, after reading a story at the 4th grade level, Dane will respond to 5 out of 7 short answer questions at 75% to an 80% accuracy with minimal mechanical errors.

By the end of 4th quarter, after reading a story at the 4th grade level, Dan will answer 5 out of 7 short answer questions at 80% to 85% accuracy with minimal mechanical errors.

1. Dan will write short story at 4th grade level questions with answers with minimal mechanical errors. And parents will monitor this and give data collected back to teacher. 1. Dan will make grocery list with correct spelling as his parents tells what they need from the store, to practice his writing without errors.

4. By the end of 4th quarter, after reading a story at the 4th grade level, Dan will answer 5 out of 10 short answer questions at 80% to 90% accuracy with no mechanical errors.

Rationale In 250-500 words, rationalize your PLAAFP choices. Be sure to address how your choices are specific to the needs of the student and consider the personal background of Dan and his family (as explained in the provided PLAAFP).

Dan and his family will be involved with improving Dan’s goals in his IEP, by the parents are already concerned about his reading oral out loud and his writing with lots of mechanical errors such as, (grammar, spelling, punctuation, capitalization), and he has difficulty following directions when to many are given at once. And his reading he is not able to read at a normal rate, he reads slow and labored. Dan at home with the help of his parents will practice his reading out loud then do a weekly 1-minute timing with his parents so see how his words per minute are and with a good 80 % percent accuracy to work towards till he can get to 100 words per minute with 90 percent accuracy in the 4th grade level reading. Then we will move onto helping him get to 4th grade. Sometimes these goals take time and a lot of practice. Dan is a 10th grader who is a good student with a sense of humor and is accepted by his classmates, and he works hard and needs a little extra help with his writing, writing and he has difficulty in following directions. And it appears he is willing to try and is not easily frustrated so this will also help with the teacher working with him as well as his parents. His parents have voiced how proud there are of Dan, and really want to help him. Then at the end of each week Dan’s parents can send

his data to the teacher to see how Dan has progressed in reaching his goals through the help of his parents along with the teacher when he is at school.
