Description of Procedure 50 g of oral glucose load, followed by plasma glucose analysis 1 hr later performed at 24-28 weeks of gestation; fasting not necessary, a positive blood glucose screening is 130-140 mg/dL or greater

Indications -Testing is indicated upon onset or recognition of impaired tolerance to glucose found in pregnant woman.

Interpretation of Findings -if positive results are found after testing with 1 hr glucose tolerance test (blood glucose > 130-140 mg/dL) then additional testing with 3 hr oral glucose tolerance test is indicated

Potential Complications -Spontaneous abortion -Infections -Hydramnios -Ketoacidosis -Hypoglycemia -Hyperglycemia

CONSIDERATIONS

Nursing Interventions (pre, intra, post) -Educate the client about steps for glucose tolerance screening test: -you do not need to prepare or change your diet in any way -you will be asked to drink 50 g of oral glucose -your blood will be drawn 1 hour often you drink the glucose solution to check your blood glucose level -Educate about the need to come back for 3 hr glucose tolerance test if the results are abnormal.

Client Education -instruct client to perform daily count kicks -educate the client about diet, including standard diabetic diet and restricted carbohydrate intake. Dietary counseling by a registered dietician should occur -educate the client about exercise. -instruct the client about self-administration of insulin -educate the client about the need for postpartum lab testing to include OGTT and blood glucose levels

Nursing Interventions -Monitor the client's blood glucose levels -Monitor the fetus
