QUESTION 1 (PO8, CO1, CO2, CO3, C4, C5, C4, C5) Petrochemicals, produced from petroleum, can be classified as olefins and aromatics. Olefins include ethylene, propylene, and butadiene which are raw materials for making plastics. Ethylene is polymerized to produce polyethylene or, in combination with propylene, to produce copolymers that are used extensively in food-packaging wraps, plastic household goods, or building materials. A catastrophic explosion and fire at the Williams Olefins Plant in Geismar, Louisiana, on Oct. 17, 2017 occurred during nonroutine operational activities that introduced heat to a type of heat exchanger called a “reboiler” which was offline, creating an overpressure event that has resulted an equipment rupture. WorkSafeBC is preparing an incident investigation report (IIR) of the incident. You are one of the investigation team and you need to prepare the report. a) In your report, apply the factors in sequence that lead to the incident as proposed by Domino’s Theory. The incident animation may guide you to prepare the report. (10 marks) b) From the investigation, it was found that the company did not implement OSHA standards. Propose three (3) suitable regulations that Williams Olefins Plant should be implemented to develop standards and recommended safety practices for olefins industry. Justify your answer. (6 marks) c) Identify two (2) potential hazards that can be found from the incident and analyse the potential hazards that correspond to the incident scenario in Williams Olefins Plant. (10 marks) d) Recommend three (3) control measures or safeguards to improve safety and reliability of the Williams Olefins Plant. (6 marks) QUESTION 2 (PO8, CO3, C6, C5, C6, C5) a) LC is defined as Lethal Concentration and the LC values usually refer to the concentration of a chemical in air. The LC50 value is referring to the concentrations of the chemical in air that kills 50% of the test animals during the observation period and could benefit in establishing occupational exposure limits for human beings. Evaluate which is the most toxicant chemical from the list below when individuals are exposed for 30 minutes and suggest one (1) most suitable type of PPE for this chemical. State all your assumptions.
