A Community Health Needs Assessment was completed by Trinity Community Hospital. In this paper, I will discuss the major risk factors identified in the assessment for the three priority service lines, if the hospital is addressing those needs, and make recommendations of how the hospital can make changes to address those needs that are not being resolved. A1. Risk Factors The health care assessment has been reviewed and several risk factors have been identified for Trinity. The risk factors are issues that exist in the community that the hospital is servicing. The community risk factors impact the services that the hospital delivers. Identifying the risk facts will help the hospital meet the needs of the community served. The major overall risk is that the number of patients that will need services is increasing, and the hospital currently doesn’t have the services or will not have the services to meet the growth. An increase in the aging population impacts hospital services with the priority on Oncology, Orthopedics and Cardiovascular service lines. Within the next five years, 15% of the population will be over 65 years old. It is expected that 50% of men and 33% of women in the community will get cancer and cancer patients are expected to increase 34%. Orthopedic cases are expected to increase 46% in the next five years with inpatient procedures expected to increase 30% and outpatients expected to increase 350%. Coronary artery disease cases are expected to increase 22%, diagnostic catherization procedures are expected to increase 22% and angioplasties are expected to increase 36%. A2. Addressing Needs Trinity is not addressing the needs identified in the assessment. With these dramatically increasing numbers, the existing resources are already stressed and will not be able to meet the expect growth. Physician practices are at capacity. Diagnosis and treatment equipment and facilities already cannot keep up with the patient volume and will not be able to meet the expected growth There is not a patient navigator program for coordination of care to schedule appointments and tests. There is little emphasis on prevention, screening and control of diagnosis and there is little to no education for lifestyle modifications. B. Health Plan Summary There are several recommendations to meet the goals that are not currently being met at Trinity. The following solutions should fill the gap of these outstanding community health needs immediately and in the future. For immediate action, one strategy is to focus on prevention. Early diagnosis with cancer screen, prevention and control will improve outcomes and lower costs. An important part of this is education. Education will build awareness and help with prevention and early cancer detection. Education should be broad for the whole community focusing on healthy living and lifestyle modifications. Education should also be specific to the three service lines. Education for oncology should include smoking cessation programs, orthopedic education should target accident prevention, and cardiovascular education should focus on cardiac risk factors.

Continuity of care is also very important. Patients need help with appointments, transferring medical records, setting up referrals, following up and helping patients find alternate care at other facilities in the communities. Existing department needs to improve their communication and coordination while a department is set up with a dedicated group of patient care coordinators. As part of the long term solution, the hospital needs obtain additional space based on the service line specifics. The Oncology department needs chemo units and operating suites. Orthopedics needs surgical suites and physical therapy/rehab capacity. The Cardiovascular unit needs additional Cath labs, surgical suites, and cardiac rehab capacity. In addition to space, the service lines need more equipment like additional linear accelerators and advanced imaging equipment. Finally, more physicians and supporting staff need to be recruited and hired....
