Precipitating Factors
ANGINA PECTORIS
Predisposing Factors
Chest pain, weakness, dizziness, SOB, and diaphoretic at rest or exertion
Pathophysiology
1. Hx of alcohol and cigarette use 2. Hyperlipidemia 3. Coronary Artery Disease 4. Hypertension 5. Myocardial Infarction 6. Diabetes
Hx of alcohol and cigarette use
Nursing Diagnosis Ineffective Cardiac Tissue Perfusion r/t myocardial ischemia AEB chest pain, SOB, and diaphoresis
Damage interior walls of arteries
Plaques builds up
Medications 1. 2. 3. 4. 5. 6.
Aspirin Carvedilol Clopidogrel Ezetimibe Pravastatin Spironolactone
Goals 1.
Hypertension Hyperlipidemia
2.
Hardening/narrowing of the arteries
Nursing Interventions
Insufficient blood flow to heart muscle
Decrease oxygen in the heart muscle
↓ cardiac output
Labs ↑ HR
3.
↑ BNP of 224 pg/mL
↑ Glucose of 114
1.
2.
Decrease tissue perfusion
↑ Troponin of 3, 858.82 pg/mL
↑ PTT of 41
Patient will verbalize decrease chest pain between 0 – 3 using the 0 – 10 pain scale. Patient will maintain maximum tissue perfusion to vital organs AEB warm and dry skin, strong peripheral pulses, alert LOC, and absence of chest pain
↑ lactic acid build up
Myocardial ischemia 4.
↓ RBC of 3.49 L ↓ Hgb of 11.5L ↓ Hct of 33.6 L Echocardiogram • •
Trivial aortic regurgitation Mild mitral and tricuspid regurgitation
5.
Assess patient’s pain rate from 0 – 10 by using the Numerical Rating Scale (NRS) and using PQRST. Monitor SpO2 to determine the respiratory function and the response to the oxygen therapy. Monitor BP, pulse, HR, and rhythm. Dysthymia can occur and further damage cardiac function. Antihypertensive medications can cause hypotensive. Administer supplemental oxygen as needed. adequate oxygen can improve contractility of the heart and reduce myocardial ischemia. Assist patient during activities especially when standing from a sitting position. Orthostatic hypotension leads to temporary decreased cerebral perfusion.
